Which guidelines are used?
The IOM 2009 (now National Academy of Medicine) ranges by pre-pregnancy BMI, plus the published twin totals. This tool assumes about 2–4 lb of first-trimester gain and applies the IOM weekly rates for weeks 14 onward.
Why does BMI category change the range?
The evidence shows people with higher pre-pregnancy BMI need less additional stored tissue for a healthy pregnancy, so recommended total gain narrows from 28–40 lb (underweight) down to 11–20 lb (obesity).
What if I am carrying twins?
Totals are higher: 37–54 lb for normal weight, 31–50 lb for overweight, 25–42 lb for obesity, with insufficient evidence for an underweight-with-twins figure. Weekly pacing for twins should come from your OB.
What are the limits?
The weekly windows are linear approximations of ranges that real pregnancies cross unevenly, and measurement error, fluid shifts, and nausea all move the numbers. Your OB provider plan overrides everything here — this is an educational estimate — not medical advice.
